Incident Narrative
On January 27, 2025, at approximately 1:45 PM, an employee was preparing to take a sample of cement from the chute of a truck as it was being poured. The employee's foot became caught between the rebar and the side wall of the footing hole. The employee was able to catch themselves on the rim of the hole, and did not fall to the bottom of the 6-foot hole. The employee sustained fractures to their tibia and fibula and was hospitalized.
