Incident Narrative
An employee was helping to remove an old fire hydrant from the bed of a truck. The employee was using a forklift to pick up the hydrant when the hydrant fell and struck him, and he fell forward. The employee was hospitalized with fractures to both orbital bones, the nose, cheeks, and jaw, and also sustained fractured and herniated discs.
