Incident Narrative
An employee was using a metal rod that was approximately 1-2 feet long to poke upward toward the product and unplug a bin that was plugged. The gate under the bin is controlled by the neighboring facility, which closed the gate while the rod was still inside it. The rod was pushed against the employee's finger, trapping it against an adjacent wall and resulting in a partial amputation through the phalanx.
