Incident Narrative

An employee was exiting a shuttle bus. As he stepped off the bus, his left foot stepped onto snow, ice, and leaves, causing his left foot slide. His right foot, which was still on the running board of the bus, began to slide as well and he fell out of the bus and onto the ground. The employee suffered an injury to his right hip and upper thigh.
