Incident Narrative
The injured employee was operating an electric-powered pallet jack, walked backward through a clear-curtained doorway, and contacted a stopped forklift operated by another employee. When the injured employee contacted the forklift, they turned around and their foot became caught underneath the forklift. The forklift began backing up, running over the employee's foot and resulting in a laceration and subsequent amputation of the employee's right little toe.
