Incident Narrative

An employee was hooked into a form system approximately 26 feet above the ground. They were hanging hoses onto concrete wall hooks on the form and turned to grab a hose from another employee standing on scaffolding approximately 10 feet below. The employee's positioning hook broke, causing the employee to fall to the ground, sustaining a fractured wrist and bruised buttocks.
