Incident Narrative

Two employees were mixing concrete in a 5-gallon bucket. A co-worker using a -inch drill with a mixing paddle attached. The injured employee was pouring dry concrete mix into the bucket from a concrete bag when the shaft of the mixing paddle caught the bag and wrapped it around the shaft. The injured employee's right index finger contacted the shaft of the drill motor which resulted in a partial amputation of the finger.
