Incident Narrative

The injured employee was driving a pallet jack. He rounded a corner as another employee on a stand-up reach truck was shifting into reverse to back up. The two vehicles collided and the pallet jack was swept into stationary racking used for storing product, causing the pallet jack driver to become pinned against the racking. The employee was hospitalized with lacerations to their right hand and a perforated bladder and broken pelvis that required surgery.
