Incident Narrative

An employee and technicians were removing bolts and pins from a crane component to be lifted and uninstalled. While removing the final pin, the technicians used a large all-thread rod as a hammer. The injured technician grabbed the all-thread rod with his left hand to guide it as a second technician thrusted. When the pin was driven free, the injured technician s left little finger entered a pinch point and was crushed, resulting in a fingertip amputation.
