Incident Narrative

An employee was finished with the job and began rigging down his equipment. The lifting sub was already on top of the testing tools and the lifting device (elevators) were latched on and ready to pick up tooling to lay the tools down on the ground. The employee grabbed the lift sub to make sure it was screwed in all the way when the lifting device came up and created a pinch point that caught their left thumb. The employee sustained an amputation of the thumb tip.
