Employees had been working on a roof laying new shingles. At the end of the day, the injured employee unhooked from his anchor point and was making his way toward the ladder to egress from the roof. He slipped on the ice and water shield underlayment and fell 15 feet, striking an aerial lift bucket before hitting the ground. The employee sustained fractures to three ribs, his nose, cheek bone, and below the eye.
