Incident Narrative
An employee had just replaced a connection from a candy cane adapter to a water hose. To investigate why water was not flowing through the line, the employee hit the switch to turn on the pump. Water pressure inside the hose caused it to burst and the candy cane adapter to fly up and hit the employee's head, lacerating their lip. The employee then fell backward and landed on the stairs of a truck, suffering broken ribs.
