Incident Narrative
An employee was installing a pin to secure an auger attachment to an excavator. As the excavator positioned the Kelly bar, the employee's left hand was caught between the first and second stage of the Kelly bar friction plates (flanges) connected to the auger. The employee sustained fractures to their index finger, middle finger, and thumb.
