Incident Narrative
An employee was on the ground level, repairing a pallet racking system that was four levels high. The vertical upright beam on the storage rack failed and a wooden pallet with plastic and aluminum parts fell from the top rack, striking the employee. The employee sustained a broken left tibia and fibula and lacerations to their head and right leg.
