Incident Narrative
The injured employee was on a ladder to check the fill level of a materials hopper on a mechanical trolley that runs on a railroad style track. Another employee was operating the hopper from a control room. When the trolley operator activated the switch to move the hopper, the injured employee's left middle and index fingers were crushed between the hopper and the track, resulting in the amputation of their left index finger.
