Incident Narrative

An employee was loading shingles on a roof from a pallet that was raised in place by a boom crane. They fell 15-20 feet from the roof to the grass-covered ground below. The employee sustained fractures to their left radius, L3 vertebra, and pelvis. They also sustained a dislocated left elbow and amnesia.
