Incident Narrative

Employee 1 was underneath a tractor and training employee 2. Employee 1 was using a cutting torch to remove a bearing when a spark from the torch ignited the oil from the drain pan. Employee 2 was removing the drain pan and slid the grate, causing it to spill on employee 1. Employee 1 was hospitalized with burns to his torso, arms, upper body, and face.
