Incident Narrative

An employee was part of a crew tasked with removing and replacing inlet grates. A grate was being lifted it into the bucket of a backhoe when the grate slipped out of one crew member's hands and fell onto the injured employee's lower right leg, resulting in fractures to their ankle.
