Incident Narrative

An employee was cleaning up the jobsite, including an onsite mixing machine. The employee removed an access cover to the auger and the auger grabbed his gloved left hand, amputating the tips of his left index and ring fingers and his little finger at the third knuckle. The mixing machine was in operation at the time.
