Incident Narrative
An employee was helping to remove an electric jack that was stuck in an empty pallet. She was standing on top of the empty pallet when the pallet jack became unstuck, causing the pallet to move and the employee to fall to the ground, striking her head. She suffered a fractured left hip requiring hospitalization and surgery.
