Incident Narrative

Two employees were removing racks of painted parts from a conveyor belt and putting them on a dolly. The injured employee had his left hand on the upright of the dolly when the other employee dropped his end of the rack. The injured employee's left ring finger was caught between the rack and the dolly, resulting in a fingertip amputation.
