Incident Narrative
An employee was using a forklift to load a truck with a cart of prehung doors. He pulled the forklift up to the back of the truck, lifted the cart up level with the rear door, and stepped onto the cart. The forklift became unbalanced and the prehung doors and the employee fell to the ground. The doors landed on his left side and dislocated his hip.
