Incident Narrative
An employee was replacing a slip sleeve under an electrical meter. He was dislodging old conduit when his demolition screwdriver got stuck in the cut. When he freed the screwdriver, it snapped out of his hands and its head struck his left eye, breaking through his safety glasses. He suffered optical nerve damage that caused him to lose all function in the eye. The employee was hospitalized.
