Incident Narrative

An employee was installing wing nuts and walers to secure a wall framework. As he was moving from one area on the wall form to another, he slipped and fell about 10-12 feet to the concrete floor, suffering a broken right leg and a dislocated right wrist. His fall protection was unhooked at the time.
