Incident Narrative
During bridge column construction, an employee was working inside a 4-foot by 6-foot column at a depth of approximately 25 feet. The employee was standing on a rebar cage to vibrate concrete as it was being poured into the column via a crane, hopper, and tremie. The concrete tremie failed, causing concrete to fall from the concrete hopper onto the employee's leg, resulting in leg fractures.
