Incident Narrative
An employee was on a forklift picking product. He picked a 4- to 6-foot piece of metal that did not fit in the cart and was sticking out of the picker. As he was driving down the aisle, the piece fell and caught on some racking, causing the shelves to break and the product to fall. Some of the product (the piece of aluminum stock in the cart) struck the employee, causing a fracture to his pelvis.
