Incident Narrative
The injured employee and another employee were removing a manhole cover. An employee lifted the cover with a hook. The hook slipped off the cover and the cover fell onto the gloved hand of the injured employee, pinning his left middle fingertip against the frame of the manhole. The injured employee sustained a fingertip amputation without bone loss and was hospitalized and underwent reattachment surgery.
