Incident Narrative

An employee was emptying a tilt hopper into a dumpster and released the lever dumping the trash into the dumpster. His left hand was resting on the fork and after the trash was emptied, the hopper rolled back and landed on his left little finger just below the nail bed resulting in an amputation.
