An employee was assisting with lubrication of machinery in a controlled area. The employee was looking up at a chain, stepped backward, and fell into the "broke hole" that rejected paper falls in to go to the broke conveyor. The employee fell 9 feet to the broke conveyor and was hospitalized for a fractured hip and separated shoulder.
