Incident Narrative

An employee on a boom lift was spotting for the placement of a steel beam that was secured to a telehandler forklift in a service elevator shaft. As the beam was being installed at ceiling height, the beam struck the opposite concrete wall and shifted toward the employee. The beam struck the employee's right thumb against the railing of the boom lift resulting in an amputation.
