Incident Narrative
An employee was standing on a 6-foot step ladder and removing a bearing plate from the tarping system on a dump truck. The plate offered little to no resistance causing the employee to lose his balance and fall from the third step of the ladder. The employee impacted the ground and fractured his left elbow.
