Incident Narrative

An employee was using a pallet jack to unload freight on a residential street. As he maneuvered the palletized freight to the trailer's liftgate, the pallet jack slid and made him lose his balance. The employee fell approximately 4 feet from the liftgate to the asphalt pavement below and sustained broken bones in his back.
