Incident Narrative

A driver was making a delivery. The customer used a skid steer to take the freight off the truck. They put the load down on the ground and took the straps off. The driver then went to throw the straps back onto the truck. The freight shifted on the forks of the skid steer and the employee's right little finger was crushed between the load and the forks resulting in partial amputation.
