On June 19, 2024, an employee was cleaning out a masonry delivery system that was attached to the raised forks of a powered industrial vehicle (PIV). The employee stepped back and fell backward to the ground. The employee was hospitalized with a head injury that required three staples, three fractured ribs, and a fractured pelvis.
