Incident Narrative
An employee was using a shredder when the shredder became stuck. The employee used a pair of scissors to unjam the machine. The machine then started back up and grabbed the scissors and pulled her right hand into the machine. The employee sustained an open fracture and partial amputation of the right middle finger with extensor tendon involvement. The employee was hospitalized and required surgery.
