On May 30, 2024, an employee was assembling pipe for a concrete structure. He was holding a 6-inch PVC pipe to align it with a hole in the concrete structure. As the pipe was pushed forward, the employee's right middle finger was caught between the end of the pipe and the edge of the hole resulting in amputation of the fingertip.
