Incident Narrative
An employee was using a forklift to move a metal pallet storage rack (24' L x 44" W and weighing 350-400 lbs.). He left the seat of the forklift to manually assist with moving the rack into place while a second forklift still had one side lifted. The rack slid off the forklift tines and struck the employee on the thighs, knocking him to the ground. He sustained a compression fracture of his L4 vertebra.
