Incident Narrative

An employee was making a delivery. He was removing straps from a pallet inside a trailer when the forklift moved, pulling the strap and the employee out from the back of the trailer. He fell 4 feet to the ground, struck his head, and lost consciousness. He sustained an injury to the left eardrum, fractures to the left side of his skull, fractures to his T10 and T11 vertebrae, and an abrasion to the right side of the forehead.
