An employee was completing an installation of fiber optic service at a customer's residence. He was connecting a temporary aerial drop to a permanent buried install. The employee had climbed approximately 6-8 feet up a 24-foot extension ladder positioned on the middle of a pole when the ladder rolled causing him to fall. The employee sustained fractures to his clavicle, right wrist, and ribs.
