Incident Narrative

Two employees were installing a drive shaft into a pick-and-pack machine during the manufacturing process. The drive shaft weighed approximately 40 pounds and measured 3-4 feet long and 1.5-2 inches in diameter. The employees were having difficulty getting the shaft into the journal, so they backed the shaft out of the machine. The shaft slipped and caught the injured employee's right thumb against the machine, resulting in a partial amputation of the thumb under the cuticle.
