Incident Narrative
An employee was working to secure a full-size enclosed pan rack, measuring 69 inches tall, that was on the liftgate of a box truck. As the liftgate was lowered, the pan rack rolled down from the liftgate and fell onto the employee, causing her to fall to the ground. The rack, which was loaded with food items, landed on her leg causing a fractured femur.
