Incident Narrative
An employee was using a hose to rinse the rollers of a glue machine that had laminated wood boards. The hose got caught in the rollers before pulling the employee's right arm into the rollers, resulting in hospitalization due to a contusion to the forearm and elbow, an elbow sprain, muscle strain, fascia and tendon damage in the forearm, pain in the forearm and elbow, and a contusion and pain in the right upper arm.
