Incident Narrative
An employee was operating an electric pallet jack in reverse to load a double-stack empty battery platter rack into a vertical reciprocating conveyor infeed. The rack struck the central divider, causing the racks to slide back toward the pallet jack and strike the employee's left hand, resulting in a fingertip amputation.
