Incident Narrative

An employee was standing on the fourth rung of a 6-foot ladder while pushing fish tape when the fish tape got stuck. As the employee shifted their weight, the ladder shifted and fell out from under them, causing them to fall head-first to the concrete ground. This resulted in head trauma.
