Incident Narrative

An employee was assisting with moving a chip conveyor using a forklift. The forklift was backing away from a trailer when the conveyor shifted and fell off the forks striking the employee's shoulder and upper body, and landing on their lower right leg. The employee sustained a fractured lower leg and was hospitalized.
