Incident Narrative

The injured employee parked a mid-center forklift adjacent to a counterbalance forklift inside a racking bay. As they exited the mid-center forklift, they were struck by the counterbalance forklift as it was backing up. The injured employee's left ankle was caught between the forklifts resulting in toe and heel fractures.
