Incident Narrative

An employee was performing the final alignment of an MSE panel installation while an excavator was parked perpendicular to the MSE wall. During the pushing operation a 4x4x4 wood block that was placed against the panel splintered and broke. When it broke, the force of the excavator bucket pushed the employee through the safety barrier causing him to fall to the ground. This resulted in facial injuries and a fractured arm.
