Incident Narrative
An employee was on a fuel truck while flushing a fuel line from diesel to gas when a flash occurred, knocking him off the top of the fuel truck. The employee landed face down on the ground and his shirt was burnt off. The employee sustained injuries to his head, including an intracranial bleed, a fractured scapula, facial and rib fractures, and second-degree burns to the face, upper chest, and arms.
