Incident Narrative
Two employees were painting the potato tub. Employee 1 was sitting on the edge of a wood plank on top of the tub when the wood plank tipped and the employee fell 11-15 feet. The plank fell between the wall and the tub and struck employee 2 on the head causing them to fall from a ladder onto the concrete floor. One employee suffered bruises to their left hip and was not hospitalized. The other employee suffered contusions to their head and neck, and three fractures to their back requiring hospitalization.
