Incident Narrative

An employee was unloading a trailer using a forklift. During the operation, a piece of slip sheet fell off the pallet and got caught in the forklift. As the forklift entered the freezer, the steering locked up and the forklift slid into the freezer racks. The employee was pinned between the forklift and the racks and suffered injury to their right leg and pelvic area.
